Ban Appeal Format

There is no format.  I know all of the information that most ban appeal formats ask for, so as long as your forums username and in-game name are similar enough, don't include any of it.  Create a thread detailing your situation and why you think you deserve to be unbanned.  Provide as much - or as little - information as you think I need to know to consider unbanning you.
